This book offers concrete ideas and examples to those interested in driving a radically different approach to supporting people with dementia and their families. "We have explored a number of approaches with people who have been leading their development. We have been keen to look at both approaches that emerge from working directly to improve support for people with dementia and others that have different roots, but we think are potentially very transferable. None of the approaches is yet being used at any significant scale". Discussions and examples are all UK based. There is an introduction detailing current problems and issues with care and support for people with dementia. 10 approaches are described for housing and support, 4 concerned with enabling people to have good days and 7 associated with enabling people to connect with their community.

CBM has joined the Global Network on Disability Inclusive and Accessible Urban Development (DIAUD) consisting of multi-stakeholder partners working on both disability and urban development issues, advocating for the inclusion of women, men, girls and boys with disabilities in the New Urban Agenda and the UN Habitat III process. 

On behalf of DIAUD network, CBM and World Enabled have produced an innovative booklet on the Inclusion Imperative: Towards Disability Inclusive and Accessible Urban Development.

The booklet is filled with examples of disability-inclusive urban development, features the voices of people with disabilities claiming their rights as well as key recommendations to help ensure that cities respond to the needs of everyone, including persons with disabilities. The publication also contains a foreword by Catalina Devandas Aguilar, the UN Special Rapporteur on Disability.

The publication will be launched on October 16th, during the high-level forum on disability-inclusive urban development and further disseminated during the conference including stakeholder’s roundtable.

Communication rehabilitation in sub-Saharan Africa: A workforce profile of speech and language therapists

Background: There is an urgent global need to strengthen rehabilitation services for people with disabilities. In sub-Saharan Africa, rehabilitation services for people with communication disabilities continue to be underdeveloped. A first step in strengthening services for people with a communication disabilities is to understand the composition and conditions of the current workforce.


Objectives: This research describes a sample of the speech and language therapists (SLTs) working in SSA (excluding South Africa). This study explores the characteristics of this workforce, including their demographics, education, experience and geographical stability.


Method: A mixed-methods survey was used to collect data from SLTs within Anglophone countries of SSA. Completed surveys were received from 33 respondents working in 44 jobs across nine countries. Analysis included descriptive and non-parametric inferential statistics. This study reports on a subset of descriptive and quantitative data from the wider survey.


Results: A background profile of SLTs across the region is presented. Results indicated that the workforce of SLTs comprised a mix of local and international SLTs, with university-level education. Local SLTs were educated both within and outside of Africa, with more recent graduates trained in Africa. These data reflected the local emergence of speech and language therapy training in SSA.


Conclusion: This sample comprised a mix of African and international SLTs, with indications of growing localisation of the workforce. Workforce localisation offers potential advantages of linguistic diversity and stability. Challenges including workforce support and developing culturally and contextually relevant SLT practices are discussed.
 

Development and functional demonstration of a wireless intraoral inductive tongue computer interface for severely disabled persons

Purpose: Individuals with tetraplegia depend on alternative interfaces in order to control computers and other electronic equipment. Current interfaces are often limited in the number of available control commands, and may compromise the social identity of an individual due to their undesirable appearance. The purpose of this study was to implement an alternative computer interface, which was fully embedded into the oral cavity and which provided multiple control commands.

 

Methods: The development of a wireless, intraoral, inductive tongue computer was described. The interface encompassed a 10-key keypad area and a mouse pad area. This system was embedded wirelessly into the oral cavity of the user. The functionality of the system was demonstrated in two tetraplegic individuals and two able-bodied individuals

 

Results: The system was invisible during use and allowed the user to type on a computer using either the keypad area or the mouse pad. The maximal typing rate was 1.8 s for repetitively typing a correct character with the keypad area and 1.4 s for repetitively typing a correct character with the mouse pad area.

 

Conclusion: The results suggest that this inductive tongue computer interface provides an esthetically acceptable and functionally efficient environmental control for a severely disabled user.

Travelling with a disability - Guideline

This guideline was prepared in close cooperation with several CBM travellers with a disability and others who travel together with a person with a disability. Their experiences/testimonies have been collated using a series of guiding questions.

Who is this guideline for? This guideline targets travellers with a disability and personal assistants (PA) of travellers with a disability. In addition, this document is useful for anyone who is travelling together with a person with a disability.

What is the objective of this guideline? The aim of this guideline is to raise awareness and help to better prepare for trips. The guideline also gives first-hand advice and best-practice recommendations from persons with a disability for persons with a disability when they face challenges during their trip.

Good practices of accessible urban development - Making urban environment inclusive and fully accessible to ALL

About 6.25 billion people, 15 per cent of them persons with disabilities, are expected to be living in urban centres by 2050. Urbanisation has the potential to be an engineer for achieving sustainable and inclusive development for all. The current lack of environmental accessibility faced by people with disabilities, in particular in many cities in the world, presents a major challenge. As the international community prepares for the Third Global Conference on Housing and Sustainable Urban Development (Habitat III), which will take place in Quito, Ecuador, in October 2016, it is an apt and a strategic opportunity to promote an accessible and inclusive Urban Agenda.

This report is a compendium of promising initiatives and good practices that have emerged in recent yeas successfully promoting accessibility and inclusion of persons with disabilities, their rights, aspiration and contributions in the contexts of urban development. It countains findings and recommendations which were adopted at a UN expert group meeting, that may be helpful in informing the ongoing Habitat III discourses, the development of the New Urban Agenda as well as in furthering accessible and inclusive urban development 

Long-term training with a brain-machine interface-based gait protocol induces partial neurological recovery in paraplegic patients.

"Brain-machine interfaces (BMIs) provide a new assistive strategy aimed at restoring mobility in severely paralyzed patients. Yet, no study in animals or in human subjects has indicated that long-term BMI training could induce any type of clinical recovery. Eight chronic (3–13 years) spinal cord injury (SCI) paraplegics were subjected to long-term training (12 months) with a multi-stage BMI-based gait neurorehabilitation paradigm aimed at restoring locomotion. This paradigm combined intense immersive virtual reality training, enriched visual-tactile feedback, and walking with two EEG-controlled robotic actuators, including a custom-designed lower limb exoskeleton capable of delivering tactile feedback to subjects. Following 12 months of training with this paradigm, all eight patients experienced neurological improvements in somatic sensation (pain localization, fine/crude touch, and proprioceptive sensing) in multiple dermatomes. Patients also regained voluntary motor control in key muscles below the SCI level, as measured by EMGs, resulting in marked improvement in their walking index. As a result, 50% of these patients were upgraded to an incomplete paraplegia classification. Neurological recovery was paralleled by the reemergence of lower limb motor imagery at cortical level". It is hypothesised that this unprecedented neurological recovery results from both cortical and spinal cord plasticity triggered by long-term BMI usage."

'Brain training' technique restores feeling and movement to paraplegic patients

It is reported that eight paraplegics – some of them paralysed for more than a decade by severe spinal cord injury – have been able to move their legs and feel sensation, after help from an artificial exoskeleton, sessions using virtual reality (VR) technology and a non-invasive system that links the brain with a computer. "After just 10 months of what the Brazilian medical team “brain training” they have been able to make a conscious decision to move and then get a response from muscles that have not been used for a decade". The work is part of the Walk Again Project.

This report was prepared to inform the discussions at the high-level political forum (HLPF) on sustainable development in 2016. The theme chosen for the HLPF is "ensuring that no one is left behind". The report builds on GSDR2014 and GSDR2015. The approach is of an assessment of assessments, documenting and describing the landscape of information on specific issues or nexuses of issues. Specifically, the report keeps the ‘science-policy interface’ and ‘SDGs as integrated system’ as main threads. Main topics include: ensuring that no one is left behind and the 2030 Agenda; the infrastructure – inequality – resilience nexus; perspectives of scientists on technology and the SDGs; inclusive institutions for sustainable development; and identification of emerging Issues for sustainable development. An annex addresses persons with disabilities specifically, highlighting their over-representation in the "furthest behind".

Disability inclusive health, safety & security management. A vital part of duty of care.

This briefing paper aims to encourage security managers and policy makers towards implementing disability inclusive safety and security protocols and standards as an integral part of Duty of Care within the humanitarian, development and private sector. Development of SOPs, guidelines and contingency plans, training, briefings, feedback and incident reporting mechanism are outlined. Examples are provided of a visually impaired person in a vehicle at a roadblock and of disability inclusive travel preparations.

Graduates’ perceptions of prosthetic and orthotic education and clinical practice in Tanzania and Malawi

Background: Maintaining and improving the quality of prosthetics and orthotics education at the Tanzania Training Centre for Orthopaedic Technologists is essential for the provision of appropriate prosthetics and orthotics services in African countries.

 

Objectives: To describe how Tanzanian and Malawian graduates’ of the Diploma in Orthopaedic Technology perceive their education and how it could be improved or supplemented to facilitate clinical practice of graduates.

 

Methods: Nineteen graduates from the diploma course in orthopaedic technology were interviewed and phenomenographic analysis was applied to the data.

 

Results: Seven descriptive categories emerged, namely varied awareness of the profession before starting education, well-equipped teaching facilities, aspects lacking in the learning context, need for changes in the curriculum, enabling people to walk is motivating, obstacles in working conditions and the need for continuous professional development. All participants perceived possible improvements to the content and learning environment.

 

Conclusions: Prosthetic and orthotic education can be better provided by modifying the content of the diploma programme by dedicating more time to the clinical management of different patient groups and applied biomechanics as well as reducing the programme content focusing on technical aspects of prosthetic and orthotic practice. Graduates were not prepared for the rural working conditions and the graduates desired continued training.

Impact of structured wheelchair services on satisfaction and function of wheelchair users in Zimbabwe

Background: Providing wheelchairs without comprehensive support services might be detrimental to user satisfaction and function.

 

Objectives: This paper compares wheelchair user satisfaction and function before and after implementation of comprehensive wheelchair services, based on the World Health Organization guidelines on wheelchair service provision in less resourced settings, in Zimbabwe.

 

Method: A pre- and post-test study with a qualitative component was done. Quantitative data were collected with the Quebec User Evaluation of Satisfaction with Assistive Technology for adults and children and the ‘Functioning Every day with a Wheelchair Questionnaire’. Data were collected from 55 consecutively sampled wheelchair users, who received a new wheelchair in the study period. Qualitative data were collected through two audio recorded focus groups and two case studies and are presented through narrative examples.

 

Results: The proportion of adult users who were satisfied significantly increased for all wheelchair and service delivery aspects (p = 0.001 - 0.008), except follow-up (p = 0.128). The same was true for children’s post-test ratings on all variables assessed (p = 0.001 - 0.04), except training in the use of the device (p = 0.052). The biggest improvement in satisfaction figures were for comfort needs (44.3%), indoor mobility (43.2%), outdoor mobility (37.2%), safe and efficient, independent operation (33.5%) and transport (31.4%). The qualitative data illustrated user satisfaction with wheelchair features and services.

 

Conclusion: The wheelchair service programme resulted in significant positive changes in user satisfaction with the wheelchair, wheelchair services and function. It is recommended that the Zimbabwean government and partner organisations continue to support and develop wheelchair services along these guidelines.

Impact of inaccessible spaces on community participation of people with mobility limitations in Zambia

Background: The study investigated the perspective of people with mobility limitations (PWML) in Zambia, firstly of their accessibility to public buildings and spaces, and secondly of how their capacity to participate in a preferred lifestyle has been affected.

 

Objectives: Firstly to provide insight into the participation experiences of PWML in the social, cultural, economic, political and civic life areas and the relationship of these with disability in Zambia. Secondly to establish how the Zambian disability context shape the experiences of participation by PWML.

 

Method: A qualitative design was used to gather data from 75 PWML in five of the nine provinces of Zambia. Focus group discussions and personal interviews were used to examine the accessibility of the built environment and how this impacted on the whole family’s participation experiences. The nominal group technique was utilised to rank inaccessible buildings and facilities which posed barriers to opportunities in life areas and how this interfered with the whole family’s lifestyle.

 

Results: Inaccessibility of education institutions, workplaces and spaces have contributed to reduced participation with negative implications for personal, family, social and economic aspects of the lives of participants. Government buildings, service buildings, and transportation were universally identified as most important but least accessible.

 

Conclusion: Zambians with mobility limitations have been disadvantaged in accessing services and facilities provided to the public, depriving them and their dependants of full and equitable life participation because of reduced economic capacity. This study will assist in informing government of the need to improve environmental access to enable equal rights for all citizens.

Disability considerations for infrastructure programmes

This study identifies and summarises evidence of the impact of non-accessible infrastructure on people with disabilities. It makes recommendations on how to incorporate the principals of universal access into all infrastructure projects. This document should be read in combination with the DFID (UK Department for International Development)​ Disability Framework “Leaving No One behind” (2014), which sets out how DFID promotes inclusion of people living with disabilities in all its programmes. Topics covered include: rationale for inclusive infrastructure; best practices in project planning, engineering design; monitoring and evaluation processes; inclusive design in planning and policy; mainstreaming disability considerations into infrastructure programmes and policy decisions; linking disabilities with cross cutting agendas.

The committee considered whether the UK Equality Act 2010, a legislative framework, adequately supports the fight against disability discrimination and how it can be made to work better for disabled people. Aspects covered include: the Red Tape Challenge; the Public Sector Equality Duty; leisure facilities and housing; access to justice; the restoration of the Equality and Human Rights helpline and conciliation service; and communication. Major issues identified were the need to include disabled people in the planning of services and buildings and communication concerning this, the need to be proactive rather than reactive or process driven and the importance of enforceable rights. Statistics concerning disabled people living in the UK are presented. The development of the Equality Act, and it's relationship with the UNCRPD and with EU law are outlined.

Evaluation of the feasibility and acceptability of the ‘Care for Stroke’ intervention in India, a smartphone-enabled, carer-supported, educational intervention for management of disability following stroke

This study aimed to identify operational issues encountered by study participants in using the ‘Care for Stroke’ intervention and to evaluate the feasibility and acceptability of the intervention. ‘Care for Stroke’ is a smartphone-enabled, educational intervention for management of physical disabilities following stroke. It is delivered through a web-based, smartphone enabled application (app). It includes inputs from stroke rehabilitation experts in a digitised format. Sixty stroke survivors discharged from hospital in Chennai, South India, and their caregivers participated in the study. The preliminary intervention was field-tested with 30 stroke survivors for 2 weeks. The finalised intervention was provided to a further 30 stroke survivors to be used in their homes with support from their carers for 4 weeks. Field-testing identified operational difficulties related to connectivity, video-streaming, picture clarity, quality of videos, and functionality of the application. Assessment was carried out by direct observation and short interview questionnaires. 

 

 

Making migration accessible: Inclusive relocation for people with disabilities

Factors associated with complex and specific needs of people with disabilities who become migrants owing to climate change are discussed and rights of disabled migrants as covered by  UNCRPD Article 11: Situations of Risk and Humanitarian Emergencies and UNCRPD Article 18: Liberty of Movement and Nationality are highlighted. The challenge of disability-inclusive planning to incorporate migrants with disabilities in a way that maintains health, physical access and necessary support throughout the migration or relocation process and once at their destination is reported. This involves maximizing accessibility of transit and infrastructure (namely temporary camps, long-term housing and public spaces); maintaining personal care and communal support networks; and guaranteeing vital health-care and social services.

Including alternative stories in the mainstream. How transcultural young people in Norway perform creative cultural resistance in and outside of school

The development of an inclusive pedagogy takes on new urgency in Norwegian schools as the student body has become increasingly culturally and linguistically diverse. Traditionally, the Norwegian school has been dominated by homogenising and assimilating discourses, whereas alternative voices have been situated at the margins. In response to this tendency, we present two transcultural students’ autoethnographic stories produced in alternative spaces to the Norwegian mainstream, that is, in a transition class for newly arrived students and on Facebook. Both spaces are perceived as contact zones in the sense that they are culturally and linguistically complex. This article illustrates how the students perform cultural and linguistic resistance towards dominant homogenising discourses as the transition class and Facebook seem to offer opportunities for constructing alternative stories. Moreover, we contend that these alternative stories offer important knowledge for conventional education contexts since they represent stories of competence in contrast to the assumed limitations of these students.
